Immunizations & Hearing/Vision/Dental/Nutrition

By state law, a valid Georgia Certificate of Immunization (Form 3231) or a notarized statement of religious exemption is required for enrollment. A valid certificate (Form 3231) must be marked with either Date of Expiration or as "Complete for School Attendance. If it is marked with a "Date of Expiration a new certificate must be submitted within 30 days of that expiration date. A medical exemption, if applicable, should be noted on the Georgia Certificate of Immunization (Form 3231) with a current date of expiration. A medical exemption must be obtained by your child's physician annually. You may request a form from your school nurse for religious exemption or you can obtain one from your local health department.
